These forms must be filled out completely for a team to be accepted for membership in the SFDA. PLEASE READ THE INSTRUCTIONS CAREFULLY AND FOLLOW THEM.
Team Captains and players must provide a reliable home address and telephone number for personal contact. (Pub addresses and phone numbers are NOT acceptable)
The deadline for returning a completed sign-up form is Wednesday, Nov. 14th, 2001. There will be no exceptions! Representatives for the SFDA will be present at Eagle's Drift In, 1232 Noriega st. @ 19th ave. (415)661-0166 from 8pm 'til midnight on the evening of Nov. 14th to accept sign-ups, sponsor fees and individual dues. Prior to the deadline you can mail the forms to:

Register no later then Nov. 5th and receive a 10% discount on all Sponsor fees!
Sponsor fees are:
$125 for the 1st team $100 for each additional team
